In this article, we will show you how to create an order form in Google Forms in just a few clicks. We also provide a basic template for you to get started. Simply follow the steps below.
Go to Google Forms. To start a new form, click on the "Blank" page, or choose a template from the template gallery if you see one that fits your needs.

Click on the "Untitled form" at the top to give your form a name. You can also add a description below the title if necessary to provide instructions or information about the order process.
Here's an example:
Form title: "Clothing Store Order Form"
Description: "Fill out this form to place an order for our premium clothing line. We'll send you a confirmation email with the invoice and delivery details."
Click on the "Untitled Question" and type in the first question or item name, depending on how you want to structure your order form.
Choose the type of question from the drop-down menu on the right side of the question.
For item selection: Use the "Checkboxes" or "Multiple choice" options so customers can select the items they want to order. Add an option for each item you are selling.
For quantity: You may ask for the quantity as a "Short answer" question and set it as a required question.
For customer information: Add questions for the customer's name, email address, phone number, etc. These should typically be "Short answer" types and be marked as required.
For special requests or additional information: Use the "Paragraph" question type, which allows for longer responses.
For example, we can ask the customer for the following information:

Tip: You can toggle the "Required" option at the bottom to ensure that the customer doesn't skip any important field.

For the product selection, you can follow this basic structure:
1. T-Shirt ($15)
Question Type: Dropdown (with options like Small, Medium, Large)
Description: Select the size you want to order.
2. Quantity
Question Type: Short answer
Description: Please enter the amount you wish to order.
To add a description, click on the three-dot menu on the bottom-right corner of the question box. Select "Description."
Here's what our example looks like:
If you want to show images of the items, click on the image icon on the right side of the question to upload an image or link to an image URL.
For videos, like tutorials or product showcases, click on the "play" icon on the right and paste the video link.
Click on the palette icon in the top right to choose a color theme or add a header image to make your form visually appealing.
Click on "Responses" at the top of the form.
Here you can see a summary of responses. You can click on the Google Sheets icon to create a spreadsheet that will record all responses.
Go to the Settings tab. Here, you can set up things like "Collect email addresses," "Response receipt," "Limit to 1 response," and whether to allow editing after submission.
Click the "Send" button at the top right when you're ready to share your form.
You can send via email directly from the form, get a shareable link, or embed the form on your website.
Before sending it out to everyone, fill out the form yourself to make sure everything is working as it should be. To do this, click the preview mode (looks like an eye).
You can use the data validation feature in Google Forms to set a minimum order quantity. Here's how to do it:
Click the + icon on the right. Set the question type to "Short answer" by clicking the dropdown menu next to the question.
We will use our earlier example:
At the bottom right of the question box, click the three-dot menu and select "Response validation."
Set the validation criteria to "Number," "Greater than or equal to" and specify the minimum number. For example, you can set it to 5.
Provide an error message that users will see if they enter a number below the minimum. Following our example, we can set it to: "Minimum order quantity is 5."
Make sure to test it in preview mode. Click on the eye icon at the top right.
